When you schedule a consultation, our highly specialized and skilled Advanced Practice Providers (NPs and PAs) will develop a customized treatment approach tailored to your specific goals. This robust plan will be implemented alongside your main cancer care team and other specialty healthcare professionals to ensure comprehensive support throughout your treatment journey.
APP Care 1 visit-
Our APP Care Service 1 Visit: includes monthly visits with two 30-minute consultations each. These sessions focus on:
- Care Coordination: Collaborating with care partners, and family members to ensure comprehensive support.
- Medication Management: Ensuring appropriate medication use and adjustments as needed.
- Specialty Referrals: Making recommendations to relevant specialists and therapists based on your needs.
- Group Meetings: Inviting caretakers to participate in meetings to enhance communication.
This approach improves communication among caregivers and reduces confusion for you and your family, ensuring a more streamlined care experience.
APP Treatment chemo education visit
- Our APP Treatment Chemo Education Visit is a comprehensive 60-minute session designed to support you through your treatment journey.
- During this visit, we will:
- Review Your Treatment: Discuss your specific type of treatment and diagnosis.
- Nutrition and Exercise: Provide guidance on beneficial foods and exercise routines.
- Decision Support: Listen to and coach you and your family through important decisions related to treatment options, nutrition, finances, and social issues.
- Side Effect Management: Teach you and your family members how to recognize and manage the side effects of cancer treatment.
- This educational visit aims to empower you and your loved ones with the knowledge and tools needed for a smoother treatment experience.
APP Nutrition counseling visit
- During our APP Nutrition Counseling Visit, we will focus on your dietary needs and lifestyle during cancer treatment. This session includes:
- Nutrition Guidance: Discussing what you can eat to support your health and treatment.
- Exercise Recommendations: Providing advice on safe and beneficial physical activity.
- Alcohol Use: Reviewing considerations around alcohol consumption during treatment.
- Diet and Medication Management: Offering expert guidance on the use of herbs, vitamins, and other dietary supplements.
